Idaho's primary care shortage area designations cover 85 percent of the state's land mass, creating structural delays in cardiac follow-up care. Patients in the 10 northernmost counties travel an average of 78 miles to the nearest cardiologist, while those in the southeast Panhandle face similar distances to neuromuscular specialists.
Agricultural workers and food processing employees constitute 14 percent of the workforce and show elevated cardiovascular event rates tied to seasonal overtime and limited insurance coverage during off-seasons. Veterans residing in rural counties near Mountain Home and the Nez Perce reservation experience additional coordination gaps between VA facilities and local hospitals.
Current electronic health record fragmentation prevents consistent transfer of care plans when patients move between critical access hospitals and larger centers in Boise or Idaho Falls. This produces duplicated testing and missed medication adjustments.
A health navigator network stationed at existing critical access hospitals and tribal clinics can intercept referrals at the point of discharge. Navigators would hold state certification for insurance navigation and maintain direct access to the Idaho Health Data Exchange. Weekly case review meetings with regional cardiologists occur via existing telehealth lines already installed for stroke networks.
The model requires applicants to document memoranda of understanding with at least three critical access hospitals and one tribal health program before submission. Navigator positions must be embedded within current clinic staffing rather than added as standalone roles to avoid administrative overhead.
Unlike Montana or Wyoming programs, Idaho applications must account for the absence of a statewide Medicaid expansion equivalent that covers transportation assistance, requiring explicit budget lines for mileage reimbursement drawn from program funds.
