Rural counties comprising 42 percent of the population lack any advanced neuroimaging equipment, forcing reliance on CT which cannot detect the required cortical thinning patterns.
The state's neurology workforce totals 38 board-certified specialists, with 71 percent concentrated in Ada and Canyon counties. Eastern Idaho and the panhandle regions report fewer than one neurologist per 75,000 residents, directly constraining enrollment in imaging-intensive FTD projects. University of Idaho and Idaho State University maintain small neuroscience departments without dedicated FTD biomarker cores.
The funding mechanism requires applicants to establish mobile imaging partnerships that rotate equipment to Pocatello and Twin Falls at minimum quarterly intervals. Budgets must include line items for technologist training at Boise VA Medical Center and data transmission upgrades that meet NIH security standards over existing rural broadband.
Eligibility criteria specify that the primary institution must already hold or commit to a data-use agreement with at least one Idaho critical access hospital. Proposals must demonstrate capacity to scan a minimum of 60 participants annually while maintaining 90 percent follow-up retention across counties separated by mountain ranges.

Capacity assessments must quantify scanner hours available after clinical obligations and detail backup plans using out-of-state facilities in Spokane or Salt Lake City. Workforce plans require recruitment from Idaho's physician assistant training programs rather than external hires.

Unlike Montana or Wyoming applications, Idaho proposals must address the specific absence of any state-funded dementia registry and the resulting reliance on private clinic records for baseline prevalence estimates.
